Position Summary:

We seek an HIV Care Services Coordinator to join the Wisconsin HIV Care Team. In collaboration with the UW Population Health Institute, this position will be based in the Communicable Disease Harm Reduction Section, Bureau of Communicable Diseases, Division of Public Health, Wisconsin Department of Health Services (DHS). This position is under the general supervision of the HIV Care Supervisor at DHS.

The HIV Care Services Coordinator is responsible for guiding the ongoing development, implementation and evaluation of a statewide system of innovative mechanisms to link people living with HIV to client-centered, coordinated care and support services. Services are provided through contracts with AIDS Service Organizations (ASOs), Community-Based Organizations (CBOs), and Federally Qualified Health Centers (FQHCs), and are supported with $4 million in state general purpose revenue (GPR) and over $4 million in federal funds annually.

Degree and Area of Specialization:

Bachelor's degree required.

Minimum Years and Type of Relevant Work Experience:

Experience working in the HIV or sexual health fields strongly preferred.

Experience with HIV case management, or knowledge of case management practice and principles especially as they relate to HIV.

Knowledge of medical, mental health, substance use and social service delivery systems especially as they relate to HIV.

Knowledge of health insurance programs, both public and private, especially as they relate to HIV.

Ability to provide project management, lead administration and policy direction for programs.

Knowledge of program monitoring, assessment and evaluation techniques.

Ability to collect and analyze data and prepare reports on client service delivery especially related to HIV care and treatment service delivery.

Ability to develop and/or adapt health education and training curricula.

Ability to develop and coordinate conferences, meetings and in-service training especially related to health education, health care and human service delivery, and HIV.

Ability to understand, interpret and effectively communicate policies and procedures orally and in writing.

Understanding of confidentiality as it applies to client medical and human service records, especially as it relates to HIV.

Ability to manage and track several projects simultaneously.

Ability to establish and maintain effective working relationships with a variety of internal and external agencies and personnel.

Knowledge of and ability to use personal computer applications, including word processing, spreadsheet and database programs to prepare documents and manage data.

Knowledge of and sensitivity to working with individuals from diverse communities, including the LGBTQ community, and persons with diverse backgrounds, including persons with a substance use history.
