General Summary/Purpose:

The Case Manager will work and partner with the Program Manager in supporting, overseeing, and motivating a team of undergraduate student advocates (volunteers) from John Hopkins University to collaborate with patients to address their unmet social needs within the primary pediatric care clinical setting primarily serving Spanish-speaking families.

Ideal candidates are detail oriented, exceptional communicators, and enjoy building strong relationships. This is a full time position reporting to the Program Manager.

Specific Duties & Responsibilities:

Manage and develop the Advocates and Student Leaders

    Supervise a team of undergraduate student volunteers to screen and address patients’ social needs Collaborate with a team of student leaders who oversee and support peers working as advocates Be a daily presence at the clinic and various evening meetings to guide advocates’ professional and leadership development Develop and remain updated on vulnerable population’s access to social benefits, and community resources that address social needs

Coordinate Case Management Support

    Support Baltimore team with logistical planning for Advocate training Observe and evaluate Advocates’ performance at clinic, keeping abreast of the most complex cases Collaborate on design of and implement standards for quality case management Monitor and facilitate Advocates’ communication with healthcare providers Undertake direct casework

Minimum Qualifications (Mandatory):

    Bachelor’s degree in related field. 1 year of progressively responsible program or case management experience. Fluency in Spanish required (written and verbal) Additional directly related education may substitute for some experience, to the extent permitted by the JHU equivalency formula.

Preferred Qualifications:

    Previous experience in a clinical and/or social benefits setting desirable At least 2 years relevant educational, professional or volunteer experience highly desired; familiarity with Health Leads model or the non-profit sector a plus

Special Knowledge, Skills, and Abilities:

    Proficiency with the Microsoft Office suite and comfort in technology-rich environments. Experience utilizing Electronic Medical Record systems. Understand the navigation of social benefits programs such as SNAP (Supplemental Nutrition Assistance Program) and TCA (Temporary Cash Assistance) Familiarity with community resources that meet the social needs of Baltimore’s vulnerable population desirable Demonstrated leadership experience Demonstrated ability to excel in a fast-paced, dynamic, diverse, and entrepreneurial environment, lead projects, meet concurrent deadlines, organize time and priorities Strong attention to detail, communication skills, interpersonal skills and ability to take initiative Demonstrated ability to engage and work closely with a wide range of individuals, including clinical providers, staff, and patients and families Demonstrated capacity to respond to and provide professional feedback

The pre-employment physical for positions in clinical areas, laboratories, working with research subjects, or involving community contact requires documentation of immune status against Rubella (German measles), Rubeola (Measles), Mumps, Varicella (chickenpox), Hepatitis B and documentation of having received the Tdap (Tetanus, diphtheria, pertussis) vaccination. This may include documentation of having two (2) MMR vaccines; two (2) Varicella vaccines; or antibody status to these diseases from laboratory testing. Blood tests for immunities to these diseases are ordinarily included in the pre-employment physical exam except for those employees who provide results of blood tests or immunization documentation from their own health care providers. Any vaccinations required for these diseases will be given at no cost in our Occupational Health office.
