The Department of Medicine, Division of Gastroenterology is seeking anInfusion Clinical Nurse. The Clinical Nurse will be responsible for making decisions regarding patient care; serves as patient advocate; demonstrates responsibility and accountability with patients, family members, other providers and support personnel regarding care for Infusion Patients. Exercises independent, professional judgment and decision making on a consistent basis. Adapts to changes in technology and software to ensure efficient office practice.

Applicant will attend our didactic curriculum includes weekly conferences, monthly grand rounds, monthly journal club, and monthly M&M conferences.

Duties and Responsibilities:

EPIC

  • Transcribe/scan lab work from providers into EPIC. Ensure labs are routed to the correct lab per patient insurance
  • Ensure that all patient orders are up to date and notify provider to obtain new orders prior to scanning
  • Scan all provided orders into EPIC
  • Change provider for each patient to infusion nurse to allow each encounter to be closed
  • Retrieve/print lab slips for order review
  • Document patient visit on progress notes, visit navigator, doc flowsheets, care teams and communication section of EPIC
  • Enter charges for each patient
  • Ongoing troubleshooting to identify ways to document better and or more efficiently

Patient Care

  • Ensure timely admission with name band, and 2 patient identifiers
  • Assess and record vital signs, weight, and height
  • Review history and screen for infections. Alert and discuss with primary provider
  • Verify allergies
  • Review current medications
  • Review orders and administer any premedication ordered. Ensure patient has taken any pre medications at home if prescribed
  • Insert IV and draw labs if ordered. Label labs immediately and print appropriate requisitions form EPIC
  • Reconstitute Remicade/Entyvio/Etc. in GSS lab per provider order
  • Hang medication to infuse over time period dictated by orders
  • Ongoing patient assessment during infusion. Adjust Remicade/Entyvio/Etc. as ordered.
  • Ongoing assessment of vital signs per protocol and as needed based on patient presentation
  • Ongoing evaluation for expected outcomes
  • Assess infusion site for signs and symptoms of infiltration
  • Monitor for potential patient infusion reactions or side effects
  • If infusion reaction occurs, nurse remains calm to ensure minimal disruption to other patients and notify provider immediately. Follows infusion reaction protocol and educates patient about treatment changes and re-infusion.
  • Ensure patient is stable prior to discharge
  • Ongoing documentation of above in EPIC

Communication

  • Flexible scheduling with patients and tracking patient appointments and scheduling at correct intervals
  • Triage phone calls from patients, notify providers about patient issues, i.e. sick, appointment changes, etc.
  • Address patient questions and concerns in a warm and receptive manner
  • Handle dissatisfied customers, both patients and providers. Attempt to make the infusion experience as positive as possible to ensure patients return.
  • Communicate with referring providers regarding any issue that may require referral changes, etc.
  • Communicate with referring providers regarding any changes in patient status
  • Communicate any changes during/after infusions
  • Provide a post infusion note to the providers via in basket
  • SIBPQ via providers in basket
  • Communicate with the Clinic Supervisor, Ambulatory Services Manager, other infusion nurses and infusion center staff regarding scheduling and patient needs
  • Follow up with no shows or sick patients. Communicate with providers as needed.

Patient Education

  • Online resources

  • Remistart/EntyvioConnect/Etc.
  • Pregnancy/pre-pregnancy weight and schedule
  • Importance of adhering to scheduled regimen
  • Discuss possible need for pre medications
  • Discuss disease process
  • Discuss what to expect with Remicade/Entyvio/Etc.
  • Discuss signs and symptoms of infections and importance of making provider aware
  • Discuss and keep sign up regarding the contraindication of live vaccines
  • Flu shot reminders discussed and posted
  • Infusing after an reaction

Infusion Room Maintenance

  • Update order sheet protocol as needed
  • Track and order supplies
  • Track and order emergency medications
  • Track and order Remicade/Entyvio/Etc.
  • Track and order tubing
  • Emergency check list
  • Order supplies like AED battery as needed
  • Emergency drug box; request replacement as needed
  • Ensure equipment in working order
  • JCAHO reviews

Minimum Qualifications:

Individual must be a registered nurse, licensed in the State of Maryland or state where practicing. Bachelor's degree in nursing or related discipline preferred. Current American Heart Association or American Red Cross CPR certification required. Must maintain current licensure and certification during duration of employment. Acceptable completion of the JHH credentialing process is required within two weeks of start date.

Preferred Qualifications:BSN degree

  • Demonstrated problem solving skills required.
  • Demonstrated ability for self-direction and initiation required.
  • Demonstrated ability to function as a leader and team player required.
  • Extraordinary customer service skills required. Basic computer skills required.
  • Knowledge of Microsoft Office applications required
  • Knowledge of EPIC and ORMIS scheduling systems preferred; knowledge of IDX

Physical Requirements:

Sitting, standing and walking for extended period. Reaching by extending hand(s) or arm(s) in any direction. Finger dexterity required to manipulate objects with fingers rather than with whole hand(s) or arm(s). Ability to move standard equipment through a hospital or clinical environment.
