Position Description:
DEFINITION:

Six (6) vacancies. This a part-time, 29-hour per week position. This position is dependent upon grant funds, which expires on December 31, 2020. In the interim, there will be multiple concerted efforts to seek additional funding.


PURPOSE:

The Community Engagement Worker primary function is to provide critical outreach, case management, well checks, assessments, education, support, and referrals to medical and behavioral health providers, substance use treatment, housing programs, and other resources as appropriate to homeless individuals in Monterey County.

POSITION SUMMARY:

The Community Engagement Worker is part of the Homeless Engagement and Response Team (HEART) and will participate in daily county-wide outreach to people experiencing homelessness in Monterey County. This position is under the oversight of the CHE Principal Investigator and the general supervision of the CHE Coordinator.

ESSENTIAL DUTIES AND RESPONSIBILITIES include, but are not limited to, the following:
  • Conduct daily county-wide outreach activities to develop relationships and build rapport with clients to assist them with finding a traditional home (apartment, house, room for rent, etc.) within the community of their choice.
  • Provide supportive counseling to individuals regarding the use and effect of alcohol and drugs.
  • Assist individuals to identify their need for treatment, provide substance use treatment referrals, and collaborate for individualized care
  • Conduct assessments to identify housing needs and referrals for social and housing services and work together with clients to identify housing needs, to identify potential barriers to maintaining housing and to develop strategies for maintaining housing.
  • Educate clients on different housing resources and options available to them. Work directly with clients to apply for and obtain appropriate services.
  • Develop and implement a community care plan for individuals enrolled in case management.
  • Provide information and instruction to participants regarding how to administer self-care and local resources available to them.
  • Follow-up with each participant at least weekly to ensure they are making progress towards their goals and provide back-up documentation in the client's file.
  • Provide individualized support that helps participants develop a plan that identifies barriers to acquire and sustain housing and improve their health and wellness
  • Network and collaborate with Monterey County area housing resources and area homeless service providers.
  • Work directly with area homeless service providers to verify homeless status of clients and to complete Continuum of Care/Homeless Certification paperwork.
  • Maintain accurate and timely documentation of progress notes, financial records, and other data necessary for client care and reporting goals/outcomes.
  • Maintain client-related data systems
  • Maintain confidential, professional boundaries with clients.
  • Conduct CARS/ VI-SPDAT (Vulnerability Index- Service Prioritization Decisions Assistance Tool), on all new clients in order to connect them to proper intervention and services. Document results, complete match initiation form.
  • Maintain documentation standards as set forth by CHE policies.
  • Ensure all program data is accurate and entered into the appropriate program documentation system as contractually required. (i.e. HMIS, internal spreadsheet, CARS, etc.)
  • Participate in staff meetings, collaborative case conferences, support team meetings, and other related meetings.
  • Must have own transportation, valid driver's license, and auto insurance to conduct job-related travel, and the ability to complete a web-based training in defensive driving.

  • Performs other job-related duties and special projects as assigned

PHYSICAL REQUIREMENTS:
Ability to: operate a motor vehicle; climb stairs; possess good communication skills; meet the requirements of the classification and have mobility, vision, hearing and dexterity levels appropriate to the duties to be performed. Capable of the following extended activities: standing, sitting, climbing stairs. Capable of the following intermittent activities: lifting 25 pounds or more and bending in the performance of infrequently performed office duties.

  • Ability to work and communicate as part of a team
  • Ability to maintain regular and consistent attendance
  • Ability to obtain CPR/First Aid certification
  • Knowledge familiarity with Person-Centered and Solution-based approaches, Motivational Interviewing, Harm Reduction and Housing First models of service delivery


Minimum Qualifications:
  • Four-year college degree from an accredited institution is desired. A degree in Nursing, Social Work, and Public Health is preferred or enrolled in a four-year college with a concentration in social services, health and/or wellness field.
  • Minimum 2 years’ experience working with homeless or other marginalized populations.
  • Bi-lingual capabilities are helpful but not required
  • Demonstrated understanding of and commitment to CSUMB’s vision statement


Specialized Skills:
This position requires flexibility in hours that may include working evenings and weekends.

Compensation and Benefits:

The hourly rate for this position is: $22.00. This is a non-exempt position and as such is eligible for the overtime provisions of the Fair Labor Standards.
