Purpose of Position

The Carroll County SUD Care Coordinator will work with referred residents of Carroll County as part of the HRSA R- CORP Implementation Grant. This person will provide additional screening; develop appropriate evidence-based treatment and recovery recommendations; facilitate health insurance application and enrollment processes for eligible uninsured residents; follow up with referred residents to ensure that all needed social support services an all other medical services (e.g., dentistry, optometry, mammography) are being provided; and train community leaders on available resources for treatment, recovery, and social support, insurance application processes, and other referrals for anyone seeking additional care or support. The office for the care coordinator will be centrally located to facilitate integration of care, but the coordinator will also be expected to meet residents where they are, including in their homes.

Primary Responsibilities

Provide additional SUD / OUD screening to referred residents of Carroll County;

Develop appropriate evidence-based treatment and recovery recommendations;

Facilitate health insurance application and enrollment processes for eligible uninsured residents;

Follow up with referred residents to ensure that all needed social support services an all other medical services (e.g., dentistry, optometry, mammography) are being provided

Train community leaders on available resources for treatment, recovery, and social support, insurance application processes, and other referrals for anyone seeking additional care or support

Link mothers from St. Elizabeth Healthcare’s Baby Steps program to additional county resources and support services for families.

Support and locate appropriate services for individuals, families, and caregivers.

Provide effective and efficient referrals among clinical partners, first responders, the justice system, social services, and other community-based organizations.

Qualifications

High school diploma and 1 year of related experience.

Strong computer skills (Microsoft Word/Excel) and attention to detail. Experience with project management software preferred.

Must be detail oriented, posses excellent organizational, written and communication skills.
