General Summary/Purpose

Under the supervision of the HIV Clinical Nurse Manager, this individual will perform duties related to medical case management as a member of the health care and support service team providing services to HIV infected individuals in the Baltimore City Health Department (BCHD) STD/HIV/TB Clinics. The main duties include case management for a select panel of BCHD HIV continuity patients.

Specific Duties & Responsibilities

BCHD Case Management:

  • Provides patient education on HIV, STD, hepatitis test results, disease transmission, and risk reduction
  • Provides general health education and support for patients
  • Case management for HIV continuity patients
  • Conduct psychosocial assessments and develop treatment plans
  • Counsel and refer patients with complex health problems in consultation with providers
  • Obtain sexual and behavioral risk history
  • Coordinate and track medical referrals
  • Triage patients presenting with acute issues, by phone or in person
  • Collect specimens and coordinate with lab when necessary
  • Administer treatments and medications as appropriate
  • Assist with prescription refills
  • Review lab work and triage to providers as appropriate
  • Provide HIV treatment adherence counseling
  • Adherence assessment/Plan
  • HIV/HAART education
  • Pill box preparation
  • Regular monitoring of adherence and tolerance to medications
  • Pharmacy monitoring of prescription completion by patient
  • Complete all charting in medical records per protocol
  • Additional administrative tasks as necessary
  • Assist with clinic initiatives as necessary
  • Help facility patient education and support groups as necessary
  • Interpret for Spanish speaking patients when medically necessary

Minimum Qualifications

Must be a registered nurse, licensed in the State of Maryland or state where practicing. Bachelor's degree in nursing or related discipline preferred.

Current CPR certification required.

Must maintain current licensure and certification during duration of employment.

Acceptable completion of the JHH credentialing process is required prior to start date.

Special Knowledge, Skills, and Abilities

  • Proficiency in medical and conversational Spanish is PREFERRED
  • Knowledge of STD/HIV/TB/HCV disease, treatment, and resources
  • Ability to work with culturally diverse patient, staff and community populations
  • Ability to work in multidisciplinary, multi-organizational team setting
  • Strong organizational skills with attention to detail and follow-through
  • Self-starter with the ability to work with minimal supervision
  • Strong leadership potential
  • Phlebotomy and injection skills

The pre-employment physical for positions in clinical areas, laboratories, working with research subjects, or involving community contact requires documentation of immune status against Rubella (German measles), Rubeola (Measles), Mumps, Varicella (chickenpox), Hepatitis B and documentation of having received the Tdap (Tetanus, diphtheria, pertussis) vaccination. This may include documentation of having two (2) MMR vaccines; two (2) Varicella vaccines; or antibody status to these diseases from laboratory testing. Blood tests for immunities to these diseases are ordinarily included in the pre-employment physical exam except for those employees who provide results of blood tests or immunization documentation from their own health care providers. Any vaccinations required for these diseases will be given at no cost in our Occupational Health office.
