Chief purpose of this position in support of above mission or goal

Opened in 2014, VCU’s Center for Advanced Health Management is an innovative approach to care design for seniors and others, with a wrap-around team of ancillary service providers augmenting person-centered care teams across clinical domains. Collectively, the teams provide a holistic continuum of care, with the ability to follow patients across time and across settings, including ambulatory, hospital, home, nursing facility, and end of life.

This position will focus primarily on skilled nursing facility care (90%), with a one week per-quarter rotation on VCU’s inpatient Geriatric Consult and Co-management Service. In the former capacity, the physician collaborates with an advanced practice provider, using a team-based approach to manage a right-sized panel of predominantly post-hospital rehab patients, in a small group of preferred SNF partners, which align with the Medical Center for quality outcomes. Together four such teams comprise the VCU Nursing Facility Attending Service, which has been in continuous operation since 1996.

Position Responsibilities

Teaching: To support teaching directly via interactions with Fellows, residents, medical students, and non-physician professionals. (Affiliate faculty appointment)

Research: To support the research mission of the Division.

Service: To provide service of the highest quality to the people of the state of Virginia and beyond.

Clinical: Anticipated clinical effort is 90%. The workload and time commitments are commensurate with existing Geriatrics faculty.

To be responsible for managing a panel of patients throughout the continuum of care. To assure that care is timely, appropriate, of high quality, and cost effective. To create and enhance systems to assure that all patients receive planned, evidence-based care and care is fully coordinated with ongoing continuity for the patient, family and caregiver. To work collaboratively with Attending Physicians, Fellows, Residents, NPs, and other staff to ensure a team-based approach to care is fostered. To understand and welcome the importance of roles as care providers, team members, and major influencers of our community’s perception of the geriatrician’s quality and service. Sharing night call (one in nine currently).

Other: (administrative duties, etc.) Submit billing and document care as required by the organization. Abide by all medical staff requirements. Participate in quality initiatives.

Required Qualifications

• Required: BC/BE in Internal Medicine or Family Medicine
• Must be comfortable working with an interdisciplinary team including nurse practitioners and pharmacists, and must integrate well with members of an extended care team.
• Must have direct experience managing complex inpatient and outpatient geriatric patients with a solid understanding of the geriatrician role and function.
• Demonstrated experience working in and fostering a diverse faculty, staff, and student environment or commitment to do so as a faculty member at VCU .

Preferred Qualifications

• Geriatric fellowship training strongly preferred
• Experience with CERNER electronic health record preferred.
